The form asks for five things: your name, email, phone, the service and a short message. The consent box lets the provider contact you about your request.
Once sent, your request goes to a provider in or near Madison. They take it from there. You still get to decide whether to book a consultation and whether to go ahead with any treatment.
When you hear back
The provider may ask a few questions about what you want and when you are available. It is a good time to ask where they are, who would see you, and how they handle consultations.
If it does not feel right, you can say no thanks. If it does, set up a consultation and bring your list of questions. See how it works for each step.
